A Debrief That Leaves Actions, Not Feelings DEBRIEF
Everybody knows the four questions. Debriefs still fail, because the fourth gets answered in the passive voice and the same point comes back at the next one.

Write what happened before writing why. Once the room starts on why, it stops saying anything true about what.
Actions
A learning point is not finished until somebody’s name and a date are on it. Anything less comes back at the next debrief.
